All necessary forms to include Asthma Action Plan, Severe Allergy Action Plan, Seizure Action Plan, Diabetes orders, Medication administration permission order, Immunization information and links to information pertaining to health/nursing services is located on this website.

Registered Nurses: Nurses in our district cover more than one school and are in each school at least once a week. The nurse at your school has regular communication with the Health Para and is available if any health care need or emergency arises.
Health Para: Your child’s school has a Health Para in the health room every day. Health Paras are NOT nursesBắn Cá Đổi Thưởng Trực Tuyến 2018. They have received training in basic first aid, CPR, and medication administration only.
Health Room: For the 2020-2021 School Year and due to COVID-19 we will have specific health room guidelines in order to be proactive in the prevention of risk of transmission of any illness to include COVID-19. Health rooms will be restricted to students only, parents will be encouraged to wait outside the health room at a designated area when picking up their student from the health room. Health room bathrooms will be restricted to health room related services only and will not be accessible to staff or authorized visitors. We will also encourage as much as possible for any documentation (medication orders, rescue medication action plans etc.) be communicated with your school nurse via email communication (see school nurse's email below indicated by school/program).
Parent expectation when called to pick up an ill student:Bắn Cá Đổi Thưởng Trực Tuyến 2018 Parents are expected to maintain most current contact information on your student's Power School information for emergency contacts. Parents are highly encouraged to respond quickly and make arrangements to quickly pick up their ill student from school.
Medication Policy: Colorado State law governs our medication policy and under District 49 procedures for administration of medications to students pursuant to school board policy JLCA, Student Health/Nursing Services and requirements. If your child must take medication during school and/or during any school sponsored events, you must provide the following.
● The school Nurse shall have received written orders for administration of the medication from the student's licensed health care practitioner with prescription authority. Also, the parent shall authorize school personnel to confer with the prescribing healthcare provider by providing written permission for medication administration to students. This includes all over-the-counter, prescription, and homeopathic medications. (The only exception is cough drops, which are supplied from home, and may be given with written parent permission. The cough drops will be kept in the health room, and your student may have up to 3 per day.) The physician may fax medication orders to the school. PLEASE NOTE: The pharmacy label applied to the medication bottle will NOT suffice for a doctor’s order.
- Medication will be admiistered by the school nurse, or by school personnel whom the registered nurse has trained and delegated the task of giving such medication. A parent is always welcome to come to the school and administer medication to his/her own child as necessary.
- Medication shall be in the original, properly labeled pharmacy container with the current date printed on the container, and all other parameters required for medication administration (student name, prescribing physician's name, drug name, dose, route, time and/or frequency).
- All non-rescue medications must be kept in a secure cabinet in the school health room. If a student must carry a rescue medication on him or her, such as an inhaler, Epi-Pen or Diabetes supplies, a physician order is required, and the student must sign a self-carry contract.
- 2020-2021 School Year, D49 will allow High School students ONLY to self-carry ONLY 1-2 doses of over the counter medication such as Tylenol, Motrin which must be in original store bought container and follow strict OTC medication self-carry contract and safety guidelines. All OTC Self-Carry contracts for High School students must be signed by parent/guardian, student and school nurse and renewed every school year.
Personal Care Items: The following items may be applied to your child if needed: Vaseline (for chapped lips), lotion, natural saline eye drops. Please inform your school nurse if you do notBắn Cá Đổi Thưởng Trực Tuyến 2018 want these items applied to your child.
Use of crutches and wheelchair: Each health room has a wheelchair for emergency use only. If your student requires temporary use of their own crutches or wheelchair, please provide a Dr.’s note to the health room and accommodations will be made: Parent must provide mobility devices for their student, these will not be provided from the health room.

School-Based Health Center
District 49 offers a school-based health center at Falcon Elementary School. Peak Vista Community Health Centers provides basic healthcare to district students and their siblings, 21 years old and younger. -

Asthma Action Plan

The plan includes guidance from your child's medical provider on how to manage Asthma episodes, triggers to monitor as well as medical orders for rescue inhalers to be administered at school.

Diabetes
Diabetes Independent Management Plan
This form is to be updated yearly or as needed for guide for students who are Independently self-managing their diabetes during school/school sponsored activities. This form is to be submitted to your student's school nurse's office/health room.
Provider Orders for Insulin Injection
This form is to be updated annually or every time there is a change by your student's medical provider for Insulin to be administered via INSULIN PEN ONLY (not insulin injection since this is not delegated by district Registered Nurses to unlicensed personnel at school). Form is to be submitted to your student's school nurse's office/health room.
Provider Orders for Insulin Pump
Diabetes Provider Order form is to be updated every school year or any time orders change for the management of Type 1 Diabetes for students on Insulin Pump management. This form is to be completed by your student's medical provider and submitted to your student's school nurse's office/health room.

Over-the-County Medication Self Carry for High School Students (only)
Effective 2020-2021 school year High School students ONLY will be able to self-carry 1-2 doses of over the counter medications on their person without medical provider order and under the strict process under this contract which will be reviwed and implemented ONLY after the school nurse has ensured student fully understands this contract.

Seizures
Seizure Action Plan
The Seizure Action Plan is to be completed by your student's phyisican every school year and turn into your student's school nurse. This action plan may also include medical orders for rescue medication to be administered at school in case of a Seizure Emergency as directed by your student's health care provider.

Severe Allergy
Allergy and Anaphylaxis Action Plan
The Allergy and Anaphylaxis Action Plan is to be completed every school year by your student's heath care provider for management of an allergic reaction. This document will also include rescue and emergency Epinephrine orders as directed by your student's medical provider. Please submit this form to your student's school nurse.
